Class waiver application form

Is the form' Request for confirmation of the applicability of the Agency’s decision on class waivers as a DOCX template, with the last update shown as 20/03/2019 on EMA’s Forms Templates still valid? Does it need to be included in request for confirmation of a class waiver which will be made via IRIS portal?
