The "affiliation template.docx' template for the registration of a company's First IRIS Industry User Admin still includes in the header the 'Account Managment Portal Request ID'. I can't find this number in the Account Management Portal at the stage of the request, when the completed form needs to be uploaded. Isn't it obsolete to mention this ID in the form anyway, as it is uploaded within the request?
Also, the addressee in the form is the EMA IT Service Desk, which is missleading, as it needs to uploaded in the Account Managment Portal.
